transfer: Sony TC-K71 (Dolby off) > Sek'd Prodif Plus > Sound Forge 8 (24 bit, normalize, track, bit depth conversion, export to .wav files) > FLAC level 8 encoding align on sector boundaries. Not burned to cdr, no EAC.
I made a transfer of the in Feb. 1998 using an unknown playback deck > Ensoniq PCI soundcard (an inexpensive soundcard, comparable to an early Creative Labs soundcard) > Easy CD Creator > cdrs and I'm sure I shared it. This 2009 transfer should be superior (better playback deck, better A>D, better cd mastering) - I noticed very little degradation of the tape.
My friend BW was the soundman at this show. A week or so after the show he lent me his master cassettes to copy. I was working in a stereo store and used the floor demo machines to make my copies, but as the second tape was copying somebody stole it! Not only that, but apparently they've never shared it because the last two songs from this show have never surfaced. Obviously, I never did a foolish thing like that again and believe it or not BW did in fact lend me master tapes again after that (though not right away).
There may be another version of this in circulation. What I found is described thusly:

The other version could be from BW's master, or it could be the same as mine with inaccurate lineage. I attribute the slight flaws to the unknown DAT and/or cdr generations.
My version, offered here, does not have the flaws described above - ie., not muffled, no pops, not missing opening notes.
