tech notes:
"Axilla" and the beginning of "Foam" were missing from the DAT.
Also, the very beginning of "Chalk Dust Torture" was missing,
and there were various spots of digi-noise during "Chalk Dust" as well.

The AKG patch source was also de-emphasized, because it had clearly been recorded with pre-emphasis.
